Filing A Claim

Selman & Company's supplement plans pay secondary to TRICARE and CHAMPVA. All claims for medical expenses must be first submitted to TRICARE or CHAMPVA for primary processing. TRICARE/CHAMPVA will issue an Explanation of Benefits Statement (EOB) when the claim is processed.

To ensure prompt processing of your claim(s), the following documents should be submitted to Selman & Company, plan administrator.

1. Itemized HCFA or UB92 billing statement. The statement must include the diagnosis, referring physician, and assignment of benefits if applicable.

2. The corresponding TRICARE/CHAMPVA Explanation of Benefit Statement.

If the provider is a pharmacy, the bill should include: the name of the drug, prescribing physician, date filled, and co-pay amount. Be sure to include the Identification Number on the itemized bill or prescription printout.
